Job Description

Position Overview:

We're seeking a compassionate and detail-oriented individual to join our team as a [Position Title]. In this role, you'll play a pivotal part in ensuring our members receive seamless access to essential services by coordinating and integrating medical and long-term care services. Your responsibilities will include conducting assessments, developing care plans, coordinating services, and providing ongoing support to our members, caregivers, and providers.

Key Responsibilities:

Conduct comprehensive assessments with members, caregivers, or providers to gather information on client status, support systems, and service needs for care plan development.
Monitor service delivery and maintain regular contact with members, caregivers, or providers through in-person visits and telephonic communication.
Authorize and coordinate referrals for services as needed.
Ensure the seamless delivery of provider services and identify any functional deficiencies in care plans.
Assist in coordinating the development of informal or voluntary services to complement the member care plan.
Collaborate with discharge planners, physicians, and other stakeholders to ensure appropriate discharge plans and coordination of acute and long-term care services.
Provide assistance to members in filing and resolving complaints and appeals.
Perform any other duties as assigned while adhering to all policies and standards.
Qualifications:

Bachelor's degree with 30 semester hours (or equivalent quarter hours) in a human services field (e.g., psychology, social work, nursing) and at least one year of experience in delivering services to the population served in long-term care; OR
Registered Nurse License and at least three years of experience in delivering services to the population served in long-term care.
Valid driver's license.
Pay and Benefits:

Pay Range: $22.36 - $38.07 per hour (Actual pay may vary based on skills, experience, and education.)
